چکیده

Considerable progress in the medical as well as the surgical management of angina has been made in the past decade. An appreciation of the pharmacokinetics of the "oldest" antianginal drugs, the nitrates, has lead to the development of effective long acting preparations and novel techniques of administration. On the other hand, calcium antagonists represent a new approach to the treatment of angina as an alternative to the now well established beta-blockers, and the two are often complementary. We now have a greater understanding of how to reduce work done by the heart (hence oxygen consumption) and thus relieve angina. This may be achieved in various ways (fig 1) such as by reducing venous return (preload) with nitrates or by reducing heart rate (beta-blockers) or muscle contractility (beta-blockers, calcium antagonists) directly. The resistance against which the Calcium antagonists

chest pain group. 3) Effective treatment: The efficacy of three standard antianginal drugs on silent myocardial ischemia was studied in 43 patients with EA using Holter monitoring. Antianginal drugs used were long-acting isosorbide dinitrate 40-80 mg/day for nitrates (17 patients), diltiazem 90-180 mg/day for calcium antagonists (13 patients), propranolol 30-60 mg/day or metoprolol 60-120 mg/da...

Underestimation of extent and severity of coronary artery disease by dipyridamole stress thallium-201 single-photon emission computed tomographic myocardial perfusion imaging in patients taking antianginal drugs.

OBJECTIVES This study evaluated the diagnostic value of dipyridamole plus low level treadmill exercise (dipyridamole stress) thallium-201 single-photon emission computed tomography (SPECT) in patients taking antianginal drugs. BACKGROUND Dipyridamole stress is the major substitute for maximal exercise in patients referred for myocardial perfusion imaging. Although antianginal drugs are common...

TEACHERS’ TOPICS Antianginal Actions of Beta-Adrenoceptor Antagonists

Angina pectoris is usually the first clinical sign of underlying myocardial ischemia, which results from an imbalance between oxygen supply and oxygen demand in the heart. This report describes the pharmacology of b-adrenoceptor antagonists as it relates to the treatment of angina. The b-adrenoceptor antagonists are widely used in long-term maintenance therapy to prevent acute ischemic episodes...
